Service coordination

Genacross Lutheran Services offers service coordination for residents of our senior living communities. We serve as an information and referral source to connect people with needed support services. In some instances, we provide care coordination.

A Service Coordinator works at each senior community to help connect residents with supportive services that will allow them to maintain their independent, self-reliant lifestyles. The services obtained by the coordinator can help seniors age in place in their homes with dignity and avoid premature placement into assisted living or nursing care facilities. Services that can be arranged include, but are not limited to:

  • Home-delivered meals
  • Transportation
  • Public assistance (Medicaid, food stamps/SNAP, and Medicare Part D prescription drug plan)
  • Home health care
  • House cleaning services
  • Assistance with medical bills or insurance claims
  • Hospital discharge planning
  • Senior legal expertise

Service coordination is a valuable resource that can help residents maintain their health, independance and happiness.
