The Genacross Lutheran Services-Sandusky Campus, formerly known as Lutheran Memorial Home, offers compassionate, personalized rehabilitation, memory support, and skilled nursing care in a home-like atmosphere designed for residents’ comfort and enjoyment.

Founded in 1979 by nine local congregations — Resurrection, St. John, St. Paul, Trinity and Zion Evangelical in Sandusky; Grace in Castalia; St. John in Bellevue; St. John in Monroeville; and Zion in Huron — the Sandusky Campus joined the Genacross family in 2005. We are a values-based Christian care community and welcome people of all faiths.

The Sandusky Campus was renovated in 2012 and redesigned for person-centered care with a warm, inviting feel. It features some of the largest private rooms in the Sandusky area, a spacious dining room, an expansive therapy gym, a multipurpose room, and a beauty shop. But it’s the caring staff and the opportunity to participate in a wide range of social activities and outings that really make Sandusky into a comfortable home that enhances the life of each and every resident.

Spiritual Support

Offering spiritual care to those living in our communities and to their families is an essential part of what we do every day at Genacross. Available 24 hours a day, our chaplains are specially trained to provide for the emotional and spiritual needs of older adults and their loved ones.

  • Campus Location

    The Genacross Lutheran Services–Sandusky Campus is located on the west side of Sandusky, Ohio, just 0.3 miles from the intersection of state routes 2 and 6.

  • Health Center

    Compassionate, professional, person-centered care is the hallmark of Genacross Lutheran Services. At the Sandusky Campus, our dedicated staff provides a wide range of medical care services. Whether the need is for short-term, post-acute nursing care and rehabilitation or for long-term care, our team of professionals places you and your needs at the center of everything we do.

    Our care center team includes a medical director, partnering physicians, a nurse practitioner, and 24-hour staff with registered nurses. These health care professionals have the opportunity to spend quality time with each individual to get a complete understanding of his or her unique situation. We combine advanced nursing competency with a genuine desire to serve and create individual care plans that promote independence and lead to better outcomes.

    We are committed to person-centered care and creating an atmosphere in which residents have choice and flexibility in their daily routines, dignity, privacy, and companionship that creates an overall sense of well-being. In addition, the environment is designed to be inviting and comfortable, with warm colors, spacious lounge areas, an abundance of natural light, and wood accents. All of these factors work together to create an atmosphere that helps residents — as well as their friends and family members — feel at home.

    Medical care services at the Sandusky Campus include:

    Skilled nursing care — Caring, professional nursing care is provided 24 hours a day, seven days a week by a staff of registered nurses, licensed practical nurses, and nurse aides. The excellent staff-to-resident ratio makes for an optimal level of care. Long-term care is provided primarily in the building’s west wing.

    Short-stay transitional care — For individuals who require short-term placement following a hospital stay, our experienced nursing care staff collaborates with our Therapy Department to provide the care needed to help patients heal and return home. Our clinical team coordinates with patients’ medical doctors to develop a specialized plan of care to meet all of the patients’ health care needs. We provide on-site case management services and home assessments prior to discharge to home.

    Complex medical care — Specializing in wound care services, our skilled nursing staff is experienced in wound VACs, pin care, surgical wound care, and pressure ulcerations. These services also include a wound care-certified nurse practitioner who visits bi-weekly and as needed. We also support many other complex medical needs, including:

    • Cardiac care and rehabilitation
    • Enteral/parenteral nutrition and other complex nutritional care
    • IV therapy (including most types of central lines)
    • Neurological and stroke rehabilitation
    • Orthopedic injury/post-surgical care
    • Pain management
    • Tracheostomy care


    Chronic disease management —
    For people suffering from a long-lasting condition that can be controlled but not cured, our staff is trained to work with patients with chronic illnesses. We develop and manage chronic disease care plans in conjunction with physicians and other clinical specialists to prevent exacerbation of the disease process and re-hospitalization. We provide comprehensive chronic disease programs, including:

    • Diabetes management
    • Dialysis management
    • Pulmonary and respiratory care management


    Memory support —
    When memories of bygone days eclipse the immediate past, home becomes more than a place. For those affected by dementia and memory loss, home needs to evoke a feeling of warmth, security, and unconditional acceptance. The Sandusky Campus provides care for individuals with moderate to severe memory loss. We provide therapeutic environments, creative activities, and well-trained staff members who help residents feel secure and valued.

    Palliative and hospice care — When end-of-life care becomes necessary, staff members work with area hospice providers to supply the medical services, emotional support, and spiritual resources required by people who are in the last stages of a serious illness. Palliative and hospice care focuses on relieving pain and other symptoms to enhance the quality of remaining life by keeping patients as alert and comfortable as possible in a familiar environment with family and friends. Hospice care also helps family members manage the practical details and emotional challenges of caring for a dying loved one.

    Respite care — When you care for someone at home, Genacross can offer you a break by providing a safe, enriching place for your loved one to stay for a few days, or sometimes longer. Our respite care services provide your loved one with engaging social activities, healthy meals, medication management, personal care, and more.

  • Rehabilitation

    Rehabilitation at the Genacross Lutheran Services-Sandusky Campus is an important part of our commitment to wellness and is especially designed for patients recovering from an illness, injury, or surgery that has resulted in a loss of strength or function. Our rehabilitation goal is your goal: to help you get better and back to your active lifestyle, doing the things you love.

    Therapy is a key component in helping you maintain your highest level of independence. We take a multi-faceted approach that helps you become stronger, improve your muscle tone and increase coordination. We also focus on the goals of self-management and prevention.

    We provide both inpatient and outpatient therapy services in our spacious therapy gym. Our multidisciplinary team of physical, occupational, and speech therapists will evaluate your needs and plan an individualized program designed to restore function, reduce pain, prevent further injury, and improve your quality of life.

    Case management and home assessment:

    We offer on-site case management services and home assessments prior to discharge. If more support is needed once you leave our facility, Genacross can help you arrange for home health care that will ensure your return home is a successful one.

    Insurance: The rehabilitation services available at the Sandusky Campus are typically covered by most major insurance plans, as well as Medicaid and Medicare. A referral from the patient’s physician is required in order to receive inpatient or outpatient services.

    Hours: The therapy clinic is open Monday through Friday and as needed on weekends to accommodate patient treatment plans.
