Service coordination

Genacross Lutheran Services offers service coordination for residents of our senior living communities; for community members through the PASSPORT Medicaid waiver program or HOME Choice program; and for individuals transitioning from hospitals, long-term care or rehabilitation facilities to a home setting. In many cases, we serve as an information and referral source to connect people with needed support services. In other cases, we provide care coordination. Certain services provided by our coordinators may require a fee.

Serving residents
A Service Coordinator is on site at each senior apartment community to help connect residents with supportive services that will allow them to maintain their independent, self-reliant lifestyles. The services obtained by the coordinator can help seniors age in place in their homes with dignity and avoid premature placement into assisted living or nursing care facilities. Services that can be arranged include, but are not limited to:

  • Home-delivered meals
  • Transportation
  • Public assistance (Medicaid, food stamps/SNAP, and Medicare Part D prescription drug plan)
  • Home health care
  • Housecleaning
  • Assistance with medical bills or insurance claims
  • Hospital discharge planning
  • Senior legal expertise

Serving residents and community clients
Genacross is a PASSPORT provider for independent living assistance and a HOME Choice provider for transition coordination and independent living services.
